If you don’t have everything listed, that’s okay, we’ll work with what you have.
1️⃣ If you are NOT on Medicare yet and are unsure if you should enroll
(You may still be working or covered by a group health plan)
Please bring:
✔️ Current group health insurance card
✔️ Employer or retiree health coverage information (premiums, deductibles, max out of pocket information)
(HR contact or benefits summary, if available)
✔️ Ssa.gov account login (if we need to apply for Medicare Part A) If you do not have one, we can create one for you during the appointment
✔️ Photo ID
This helps us determine whether enrolling in Medicare now makes sense or if delaying Medicare is the better option.
2️⃣ If you are turning 65 and planning to go on Medicare
Please bring:
✔️ Ssa.gov account login (To apply for Medicare Part A&B) If you do not have one, we can create one for you during the appointment
✔️ Driver’s license or photo ID
✔️ Current insurance card (if applicable)
✔️ List of doctors you want to keep (primary care and specialists)
✔️ Current medications (name and dosage if available)
✔️ Preferred pharmacy (CVS, Walgreens, etc.)
✔️ Bank Routing & Account Number (For setting up premium payments)
We may complete your Medicare enrollment during this appointment.
3️⃣ If you are already on Medicare
Please bring:
✔️ Medicare card
✔️ Current insurance cards (Medicare Supplement or Medicare Advantage plan, if applicable)
✔️ List of doctors you want to keep (primary care and specialists)
✔️ Current medications (name and dosage if available)
✔️ Preferred pharmacy
✔️ Questions or concerns you’d like to review
